AKSYS

A. If you already are signed up with AKSYS for Tax Year 2004 you do not have to sign up for Tax Year 2005. Your original signup will continue into the new filing season unless you notify us that you wish to terminate service. If you do not terminate, we will assume that you confirm your existing registration with AKSYS, and that you agree to the terms and conditions in the Operating Instructions. If any signup information such as ETIN or the name of the contact person, company telephone number or address has changed, please notify us by e-mail or telephone so that we can update our records.
You will keep the same access code to the dial-up system and, unless your telephone number changes, the web page address at this site that you used during the past filing season. You can change your access code at any time by calling G A C Operations at 1 800 231-6137. Except in unusual circumstances, your web page address can not be changed unless your telephone number changes: The web page address is based on your telephone number.
By signing up or by confirming your signup status with us, you will agree to the conditions of AKSYS Specification AK-050195, Volume 2, Operating Procedures, pertinent excerpts of which are listed under "Operating Procedures" on the homepage.
Beginning with the first acknowledgement it receives from a revenue department, AKSYS will charge you sixteen cents for each acknowledgement processed for you. Transmitters that have had their AKSYS services interrupted because they have failed to pay a bill by the due date can be charged at a higher rate.
B. If you are not signed up with AKSYS and you want to sign up, provide the following information to G A C Operations by email or phone (see the next section for the telephone number and email address)
(1) name, business telephone number, and email address of the individual who will receive bills and information from AKSYS for your company.
(2) name and mailing address of the company.
(3) indicate whether or not you want your current file of acknowledgments to be posted also at this site.
(4) all of the ETINs that will be in the acknowledgements that AKSYS will receive for you from revenue departments.

AKSYS is a service of STARPATHS INCORPORATED operated through G A C Operations, a center that combines the operations of all of our activities and companies for greater efficiency. Checks sent to AKSYS, whether to deposit funds or to pay a bill, must be payable to STARPATHS INCORPORATED

changes. Posting changes and new information at this site is the most efficient means of
providing you with publications, general information, changes, and new
procedures.
2. AKSYS will continue to honor the following privacy policy.
AKSYS
will use personal or company information that you provide in your request for service
(registration) to:
AKSYS and its parent company, STARPATHS INC will not release information we receive from you to any other company or individual, nor will AKSYS or its parent company use the information for any purpose other than those listed in 1 through 4 above.
3. AKSYS operates on a real-time basis year-round. Within a few minutes, AKSYS will process and post to transmitters' current files all acknowledgements that it receives. Acknowledgements are reposted within a few minutes of the time a transmitter requests a reposting on the dialup system.
4. All transmitters have a page at this site. At his page, a transmitter can request reposting of acknowledgements, and view and download his current acknowledgement file if he has requested at signup time that we post his acknowledgements at this site. All transmitters will continue to have a standard file on the dialup system.
5. Extracts of the Operating Procedures are available for downloading at the homepage. We will no longer provide the complete manual except to revenue departments.
